Include the following documents:
- the registration form
- Testing Accommodations Request Form
- disability documentation, if applicable
Send all completed forms and documentation to:

If you have previously received accommodations from ETS and your accommodations approval is still current, you may request the same accommodations again without providing disability documentation. The accommodations ETS previously approved for you will be approved again if they are appropriate for the current test.
If you have received testing accommodations on another standardized test such as the ACT®, SAT®, GMAT®, LSAT® and/or MCAT®, etc., you may request the same accommodations for an ETS test by submitting a copy of your approval letter. No disability documentation is required. The accommodations approved for you by another testing agency will be granted if they do not violate test construct or test security.
You may expedite the processing of your request by submitting only:
- Parts I and II of the Applicant Request for Testing Accommodations form, found in the Bulletin Supplement for Test Takers with Disabilities or Health-related Needs
- the test program's registration form
- approval letter from another standardized testing agency (for non-ETS tests)
ETS will send you an approval letter confirming the accommodations which have been approved for you. The approval letter will also outline how to register for a test with accommodations.
Don’t schedule to take a computer-delivered test until you receive your approval letter. You should follow the instructions on your approval letter for scheduling and registering for a test.
If you’ve been approved for the following accommodations, you can register for and schedule your test online through your Praxis account. You don’t need to complete and submit the Test Authorization Voucher Request Form.
- Extended time
- Extended breaks
- Screen magnification
- Selectable foreground and background colors
Retaking the same test
- If you were previously approved for accommodations on a Praxis test, review the approval letter to determine if your accommodations approval has expired. If it hasn’t, you may re-register for the same test by following the directions in the letter.
- If you need to re-register for a test with the same test name, but the test code has changed, contact ETS Disability Services at disability.reg@ets.org for approval before registering for the test.
Taking a different test
- If you’ve already been approved for accommodations on a Praxis test, and you want to use the same accommodations for a Praxis test with a different test name and test code, submit the following documents to ETS Disability Services at disability.reg@ets.org for review and approval before registering for the test: a)Parts I and II of the Testing Accommodations Request Form (PDF); and b) The Test Authorization Voucher Request Form (PDF).
Extended test time for test takers whose primary language isn’t English (PLNE).
More information is found in the Praxis Information Bulletin. ETS requires that test takers submit:
- Certification of Documentation: This form must be completed by a qualified professional (ESL teacher, foreign language department head, etc.) who attests to the fact that the test taker’s primary language is not English (page 32)
- Eligibility Form: This form must be completed by the test taker (page 33)
- Test Authorization Voucher Request Form: This form is also completed by the test taker and provides ETS with key information needed to fulfill the request (e.g., the test taker’s identifying information and the tests needed) (page 31)
